There could be a link between levels of air pollution and cardiac arrest, a new study has found.

An increase in particulate matter smaller than 2.5 microns (PM2.5) was correlated with a more than 10 per cent increase in the risk of out-of-hospital cardiac arrest, the study showed.

Sources of an increase in particulate matter include car exhausts and wood burning.

However, there could be other contributing factors to these results.

“Actually Monday at 9am is the worst, so it can be partly stress-related,” Dr Lahn Straney of the DEPM's Aus-ROC told Science Network.

“If you look at the levels of pollutants, it kind of corresponds with traffic time as well.”

Nevertheless, he says that high risk people should try to reduce their exposure to air pollution.

“A lot of carbon monoxide comes from traffic pollution and PM2.5 is affected by traffic but also burning of things, so burning wood, bushfires and things like that,” Dr Straney said.

“If we think of people that are at high risk, so older people typically, you really don’t want to be building nursing homes, you don’t want to be having hospitals on major roads.”

The study, authored by Dr Straney, looked at 8551 cases of out-of-hospital cardiac arrest in the metropolitan area attended by paramedics between 2000 and 2010. It then compared them with hourly air pollution levels recorded at seven monitors.Anonymousnoreply@blogger.com2tag:blogger.com,1999:blog-6550268593760249577.post-37748504903593614602013-11-28T16:24:00.000+11:002013-11-28T16:24:41.274+11:00Bridging the gap between research and practice. Could social media be the missing link?

The purpose of all clinical research should be to improve outcomes,
yet unless this research is effectively translated into practice by clinicians,
even the largest studies can become useless.

Gaps between evidence and practice are becoming more widely
recognised in all areas of medicine, and as a result health research funding
bodies often require grant applicants to provide a plan for the dissemination of
the knowledge. It is currently estimated that published research takes 17 years
before it is put into practice.

Dr Dashiell Gantner
of the DEPM believes that social media could represent a much faster way of
translating this research into practice. Dashiell theorises that most changes
in practice and teaching occur during hospital corridor conversations and
bed-side training. The use of social media by researchers has been show to
increase the number of times a paper is cited, and from this Dashiell believes
that it could be used to win the hearts and minds of clinicians, encouraging
the use of new procedures and treatments.

Whilst there are benefits, there are still possibilities
that research could be translated without a basis of proper evidence. As such,
care must be taken, and a structured approach to knowledge translation which
involves social media could represent a revolutionary way of bridging the gap
between knowledge and practice.

The Australian Commission on Safety and Quality in HealthCare has selected DEPM's Clinical Registry Unit, headed by Dr Sue Evans, as their preferred supplier for services in health information and data analysis, research and literature review and health information systems and web development.

Quality clinical data registries are being established progressively around the globe. They are widely recognised as a cost effective way to measure and benchmark outcomes of treatment, and provide hospitals and clinicians with feedback about performance in treating a growing number of conditions, which in turn results in improved patient outcomes, reduction in adverse events and cost savings.

DEPM has played a pivotal role in the establishment, management and analysis of clinical registries in Australia, and houses many clinical quality registries including The Australian Society of Cardiac and Thoracic Surgeons (ASCTS) Database, the Australian Cardiac Procedures Registry, The Bi-National Burns Registry (Bi-NBR), the Breast Device Registry, the Prostate Cancer Registry, the Victorian Lung Cancer Registry and the Victorian State Trauma Registry (VSTORM).
In a letter to the editor of the Medical Journal of

In an attempt to redress this dearth of knowledge, Dr Roisin
and colleagues have established the National Registry of Antipsychotic
Medication in Pregnancy. From their data they have discovered that pregnant
women are using antipsychotic medication for a wide range of disorders, and
with the use of these drugs extending beyond psychotic disorders, it is
important to understand the potential effects of antipsychotics on pregnancy.

pain was ranked as one of the leading causes of disability in Australasia,
second only to cancer. There has been immense interest and controversy
surrounding the novel study which reported the effectiveness of antibiotic
treatment for patients with low back pain. While some leading international
spine surgeons believe that this work is worthy of a Nobel Prize, two
commentaries in the BMJ have indicated that further research is needed before a
change in medical practice.
Reducing Events in the Elderly (STAREE) trial are on the winners list. The trial
will be the first randomised controlled trial of statin therapy in an apparently
healthy elderly cohort living independently in the community and will be administered through the SPHPM. Earlier trials
have shown the cardiovascular benefits to younger people with established CVD
or at high risk of CVD no studies thus far have focused on the elderly.

Amongst the issues likely to influence the balance of the
risks and benefits to the elderly are: myopathy, diabetes, cognitive impairment,
cancer and polypharmacy. STAREE will determine the effects of statin therapy
versus placebo on overall survival (all-causes of death) or disability-free survival
(need for permanent residential care) over an average 5-year treatment period
amongst elderly Australians equal to or older than 75. The study will be
conducted in Australian general practice and utilise the resources and
expertise in community-based research generated by the ASPREE study. Given the
demographic transition to an older population and the increasing health care
costs incurred by governments and individuals, this study could possibly
influence health expenditure and change the way that preventative drugs are
evaluated in this age group.

socio-economic and ethnic differences among pre-schoolers in Sri-Lanka. Vajira Nanayakkara and colleagues from the DEPM has investigated the links between
factors such as parental education levels, ethnicity and socio-economic strata
and dental health in an attempt to identify the most at risk populations.

What Vajira discovered was that compared to fathers who had a
tertiary education, those whose fathers did not study beyond grade 5 had more
dental caries and experienced worse oral health. which is linked to quality of life. Conversely,
lower levels of education among mothers were associated with better dental
health. This was reasoned as being a consequence of unemployment, and the
mothers having more time to supervise the children’s dental habits.

Tamil children also fared worse of gingivial bleeding and
poor oral health linked quality of life as compared to the Sinhalese group,
with Muslim children also having poor oral health as well. Children of lower
income families also had higher levels of gum bleeding than those from higher
income families.

From these results the researchers determined that any
future interventions should focus on children of fathers with low educational
levels and minority groups in order to reduce inequalities in oral health in
Sri Lanka.

consequence often experience a range of medical complications towards the ends
of their lives. Current practices in end-of-life medical care require major
change to ensure more Australians experience ‘better’ deaths, according to a
panel of medical, ethical and legal experts.

Following a one-day forum sponsored by the Schools of
Population health at Monash University and The University of Melbourne earlier
in the year, the panel issued a comprehensive discussion paper and blueprint
for change.

Head of Monash University’s School of Public Health and
Preventive Medicine,
Professor John McNeil said the forum had been organised to address the
changes wrought by an ageing population and rapid technological advancement
that now enables doctors to treat many medical ailments into advanced years.

“Therapeutic and technical advances have improved and
extended lives of many Australians, but at a cost - a new older generation of
people is living with more complex disease and gradual decline in quality of
life.” Professor McNeil said.

“This creates new challenges for medical practice in particular;
balancing the imperative to ‘cure’ with the personal, social and financial
burden it can create. End-of-life care implies an awareness of this balance.”

Australia has seen a large increase in the numbers of people
dying in hospital, with 52% of people dying in hospital, despite the fact that
most would prefer to die at home. In 2011, more than 146,000 people died in
hospital in Australia.

As these numbers are unlikely to change without intervention,
the panel hoped that through education, change of clinical behaviour, clinical
leadership, practitioner confidence and hospital procedures they would be able
to influence the future direction of end-of-life care.

“The benefits of improving the system are multiple -
extending to patients and families where there will be improved quality of
life-before-death, medical teams and carers to alleviate psychological, moral
and emotional distress and conflict, and an improved allocation of resources
within the health care system more broadly,” Professor McNeil said

Dr Janet Bray, under the SPHPM Travel Grant scheme, attended the European Resuscitation Council Conference (on May 25th and 26th) in Krakow, Poland. Janet also attended an informative pre-conference workshop on post-arrest care and therapeutic hypothermia.
At the conference Janet was given the opportunity to present her two submitted abstracts at a lunchtime poster session which generated interesting discussion and considerations for future research. These abstracts are published in the top-ranking international journal 'Resuscitation'. The workshop and conference also provided excellent networking opportunities, which are likely to lead to future collaborations and the opportunity to participate in the International Liaison Committee on Resuscitation’s (ILCOR) evidence review process of resuscitation science which informs national and international resuscitation guidelines.Anonymoushttp://www.blogger.com/profile/12291719289252132867noreply@blogger.com0tag:blogger.com,1999:blog-6550268593760249577.post-41770694604219874542013-11-14T15:40:00.002+11:002013-11-14T15:40:28.930+11:00Can nanoparticles cause health problems?

Adjunct Professor Anne Abbott, from Monash Department of Public Health and Preventive Medicine and Baker IDI Heart and Diabetes Institute, has spoken out about the tough climate for scientists working in Australia.

Anne was recognised for her outstanding work in stroke prevention at the Centenary Institute Lawrence Creative Prize ceremony in Sydney on Wednesday 13th November, receiving $5000 to pursue her research into stroke prevention.

She says that the prize money is invaluable to young researchers, who may receive little support from elsewhere.

Anne’s research has shown that lifestyle and medication are more effective, safer and cheaper than surgery or stenting in reducing stroke risk in patients with symptom-free carotid artery disease. “I discovered that publishing my results in 2009 was not enough to change existing health policy or practice,” Anne said. “So I started an international campaign and have had success in initiating improvements in stroke prevention standards around the world.”

“ I've been working the last 12 months without an income and I've only done that because the funding I have received so far has allowed me to become the world leader in my field of stroke prevention,” she told ABC’s the World Today.

“I'm making tremendous gains at the moment. So if I stopped now and because I am a leader, nothing would happen. So this prize money actually does help. Every little bit helps.”

The prize aims to encourage Australia's best young biomedical researchers to stay and build their careers locally.

In the current political atmosphere, moving overseas or into a more highly paid job can be a more attractive option for many young scientists.

In the past two months, the Abbott Government removed the position of science minister from Cabinet, scrapped the Climate Commission, and announced it was slashing hundreds of jobs at the CSIRO.

Actions such as these have signalled a worrying trend away from supporting the sciences in Australia.

Overnight medical staffing of acute hospitals is difficult.
Australia and the UK have witnessed an evolution in the blend of resident
personnel at night. Both countries now
have overstretched and failing Out-of-Hours (OOH) systems and there is
widespread anxiety in the UK that the current experience is not a happy one.

The failings have been identified as a lack of qualified ICU
consultants and an oversupply of under supervised junior doctors.

These junior
doctors are expected to prioritise clinical and non-clinical duties effectively
and work co-operatively while managing patients across a range of specialties. This
in turn has led to a reluctance of junior doctors to contact the on call
specialists, resulting in an overly self-reliant culture when it comes to
decision making.
The HCL is a resident senior trainee, typically a
post-fellowship doctor in intensive care medicine (ICM), often dual certified
with medicine or anaesthesia. ICM specialists are ideally suited to this role;
they are experts in the management of acute medical problems and have ample
experience in resource allocation, time management and decision making. Their
role as a HCL is to act as an initial point of contact, to provide leadership
and strategic direction to the junior doctors, allowing them to focus on their
clinical work.

This new system, while not a possibility for all hospitals,
has the potential to revolutionise the delivery of OOH care.
